That sounds like great advise, but how does one point the toes out
with the feet straight. My shoes tend to keep my toes pointed in the
same direction as my feet, straight ahead while wheel walking.

I believe John is suggesting that everyone should point their toes
straight ahead so the feet are also pointed straight ahead while wheel
walking. (This is particularly important for backward wheel walking
in which some people actually learn to do it with the feet and toes
pointed inward; learning with the toes and feet straight ahead is a
big advantage, since it is simply the superior foot position for wheel
walking backward or forward.)
